Become A Member at Karuna Health Foundation

Karuna Health Foundation proudly provides safe access to high quality medical marijuana for qualifying patients. Those who wish to become a member have several options when applying, though it should be understood that applications are largely considered on a case-by-case basis due to current legislative guidelines.

We require all applicants to complete a Membership Application form as well as a Terms and Agreement form and a code of conduct form as well to achieve membership, in addition to providing two pieces of ID (at least one must be government issued with a photo) and approved documentation of your ailment to validate your use of medical cannabis. If you are applying for mail order services please carefully review the Member Checklist.

If you are planning a visit to your own accredited Medical Doctor, Naturopathic Doctor or Practitioner of Traditional Chinese Medicine to speak with them about treating your ailment with medical cannabis, please feel free to print off the Statement of Diagnosis for them to complete on your behalf. You may also bring a copy of the Release Form for Medical Practitioners to such an appointment; this document relieves said practitioner of legal responsibility for signing a form that acknowledges your use of medical cannabis in relation to your ailment.

Download and fill out the following forms. Review them carefully and then bring them into the office.

Who is Eligible to use Medical Marijuana?

Until March 31st 2014 the only way to legally protect yourself was through Health Canada’s Marijuana Medical Access Regulations (MMAR) program. This program was discontinued and replaced by a different system wherein medically approved patients are able to purchase medical marijuana through certain designated independent suppliers and are no longer allowed to choose authorized private growers, though in many cases we may accept a recently expired MMAR/MMAP document if supported by current medical documentation of your ailment.

Karuna Health Foundation has proven time and time again that we hold ourselves to a high, thorough standard within the medical marijuana community and are happy to offer membership and safe access to the highest medicinal grade cannabis available for qualifying medical patients.
